Cancellation/ No Show Policy

Cancellation/ No show Policy:

If appointment is not cancelled or re-schedule at least 12 hours in advanced or if the appointment is missed completely with no communication with the office the patient will be subjected to a $50 fee. 


For Sports Performance: 

The client has at least 24 hours to freeze, postpone or cancel the training sessions. If client is more than 15 minutes late to a 45 min session or does not notify in given time, the session will be considered a "no show". The client will be responsible for the full charges and fees applicable to the session. 

Account Balances: 

We require payment at time of service. If the balance is not paid at the time of service, you will be subject to third party reimbursement rates. Patients/Clients with balances must make payment arrangements being made.
